First Visit

When you arrive for your first visit, please be prepared to complete a form with your health history information and any current medications you may be taking. We will ask you to fill out only the forms necessary to begin your dental treatment. We hope the process acquaints you with our office and our policies and procedures. We also hope to learn about your needs, desires, and concerns related to your dental health.

If you are covered by a dental insurance plan, please bring your card and/or a claim form, which will allow us to accurately submit your claim, electronically in most cases. We are happy to provide this service.

Your initial comprehensive exam will last approximately one hour. Oral hygiene instructions will be provided along with suggestions to help you care for your teeth. In most cases, we will also clean your teeth and take necessary x-rays during this visit and provide an evaluation that will document your existing dental conditions. Accumulating this information allows us to propose a treatment plan that will meet your needs.
